Total Chordal Preservation is a technique to replace the Mitral Valve after preserving the whole valve apparatus. Preservation of the apparatus helps maintain geometry of the Left Ventricle. also LV remodeling is faster after MVR. The patient also improves functionally faster than conventional valve replacements. This particular patient is a 41 year old with Rheumatic Heart Disease and Severe Mitral Valve Regurgitation for last 25 years. He was in NYHA FC I for last 1 year. His CTR in CXR was 90%. Echo showed large dilated left ventricle with a LA size of 8cm. EF was 30%. By AHA guidelines, he was not a good candidate for surgery, but after operation with this technique he has improved considerably and is in Functional Class I after 1 year of follow up.
