This is my current way for the intravitreal Anti-VEGF, Avastin, Lucentis, VEGF-Trap, Triamcinolone. Used for AMD, CRVO, BRVO, DME CME..etc This is a completely painless way to the patients, to the extent that some patients does not know that I already gave them the injection.
Preop: Tetracaine 0.5 % Vigamox or Zymar then Betadine 5%. Wait 15 minutes then Repeat the Tetracaine 0.5 % Vigamox or Zymar.
OP: No speculum needed. Pull the lower lid with your little finger, or patient can do that. Push the upper lid with the needle cover. Use the needle cover as your caliper, 4mm behind the limbus, in the lower nasal quadrant, which is the least painful.
Postop: Vigamox or Zymar q2hr for 24hrs then q4hr for 48hrs. (Not during sleep). Performed by Dr. Tarek Youssef.
