Trans Urethral Resection In Saline TURis
The effect of hydrostatic pressure alone in controlling arterial bleeders
Quite red due to excessive bleeding
At this point the irrigation outflow is stopped aiming to increase hydrostatic pressure within the prostatic cavity.
High hydrostatic pressure will counteract the arterial blood pressure driving the blood out of the cut vessel into my field of view.
As the out flow is obstructed the hydrostatic pressure still rises
Hydrostatic pressure continues to build up
The arterial jet is greatly subdued as the hydrostatic pressure forces fluid into the vessel
Outflow opens up again and operation continues
Low pressure vs High pressure
High hydrostatic pressure of the irrigation fluid is quite harmless during TURis
On the contrary high pressure irrigation should be avoided in monopolar resection as it greatly increases the risk of TUR syndrome
