This schematic shows the nasal tip anatomy, namely the lowe lateal catilages on eithe side. It is hee whee we'll be placing ou incision. This incision shows whee the incision line ae placed. You can see that it is the small stip of catilage above these incisions that is the tissue that we will emove. Now let's go ahead and see the inta-opeative video of the cephalic tim being pefomed. This patient has a quite bulbous tip an you can see that we ae making the incision fist on the ight side in that aeas of the lowe lateal catilage like I descibed. Once the incision is made we then continue to tim the catilage off of the undelying nasal vestibula mucosa o skin lining. We continue to emove this tissue and ultimately we'll be able to a less bulky appeaance to that side of the tip. Next we'll epeat the pocedue on the left side as well. People often wonde how much catilage is left and how do you figue out how much catilage to emove. I'm quite consevative in this pocess and the main idea is to maintain equal amounts of lowe lateal catilage once you'e done. So since sometimes thee ae asymmeties in how much is thee doesn't necessaily mean that we'll be emoving the same amount on eithe side. I typically leave at least 8 millimetes at the widest point of the lowe lateal catilage and appoximately 5 millimetes towads the domes. If additional efinement of the tip is needed I'll use additional maneuves to efine the catilages including dome binding o lowe lateal cual stut gafts. Today I'm going to talk about how we e-beak the nose duing hinoplasty using osteotomes. This figue shows whee we actually place the bone cuts to make the contolled bone factues. The inne path is the medial osteotomies and the lateal osteotomies ae on the outside potion. And hee is an inta-opeative view of me making medial osteotomies to help staighten a patient's cooked nose. You can see that the osteotome is a small device and it is essentially a vey fine medical gade chisel. We vey incementally and in a vey contolled fashion makes these bone cuts to fee up the bones in the midline. We make the cuts fist on one side and then epeat the cuts on the othe side to fee up those bones as well. Hee's a photogaph of an osteotome. This is a cuved, guaded osteotome that we usually use fo lateal osteotomies. And hee we ae now making the lateal osteotomies in the same patient. We can see that the sugical assistant uses a mallet to help me tap the osteotome. As you can see thee is not a whole lot of foce needed to do the bone cuts. The osteotomes ae quite shap and allow us to mobilize the nasal bones in a vey contolled fashion. Many people pesume that we e-beak the nasal bones in a less efined way. Pehaps just using a hamme to hit the nose o something like that? One goal of this video is to show how hinoplasty sugeons actually do e-beak the nasal bones. Pio to doing the lateal osteotomies I do use a diffeent sugical tool to fee up the soft tissue aound the bone to lift it off of the bone. Today I'm going to talk to you about how we inset ala batten gafts. Specifically, I'm going to show you the endonasal, o closed, appoach to inseting gafts today. Hee you can see that we've stated by making what is called a maginal incision on both sides of the nose. You can see how this is placed essentially in the oof of the nostil and actually bodes the lowe edge of the lowe lateal catilages on both sides. Once I'm made the initial incision I'll then use a small pai of shap scissos to make futhe dissection in that aea. The idea is to loosen the attachments of the tissue in the aea of the lowe lateal catilage. We'll then dissect a small path o tact down into the supa ala cease. Once this pocket is made we'll then be able to inset the gaft into the pocket. You can see that I fist tied and it wasn't quite lage enough so I did some futhe dissection. And now we'll ty to place it again. You can see that it fits a little bette now. The idea is to make a vey pecisely sized pocket so that the gaft doesn't actually have anywhee to move. I don't actually typically sew in the catilage itself. We'll now move onto the opposite side and do the same exact type of dissection using a maginal incision as we'd made on the left side. You can see how I've made some peopeative makings with a making pen on the outside pat of the nose to denote whee the aea of maximal weakness is. This helps me plan the pocket dissection late on duing the sugey. You can look at anothe youtube video whee I show how I actually cave these ala batten gafts. Now that the gafts ae inseted we'll then close the maginal incisions. I typically use a chomic sutue which is dissolvable. Nomally it will dissolve on its own in a few weeks on its own. I also use inteupted sutues meaning it isn't a continuous line of sutues o a baseball stitch, but athe thee ae seveal inteupted individual sutues along the shot incision line. I'm now placing anothe set of sutues to close the incision. This appoach of closed ala batten gaft placement is a vey good way to futhe impove nasal beathing issues and nasal obstuction. It is often combined with septoplasty and o tubinate eduction as it was in this patient's case. The goal of the ala batten gafts is to stengthen a weakened o collapsed nasal sidewall in the aea of the extenal nasal valve. People who find that beathe ight stips help them with thei aiflow ae often good candidates. Today I'm going to show you how to make an Asian hinoplasty dosal augmentation gaft using catilage. In this specific case we'e going to be using a mix of both septal catilage and ea catilage fom a patient. Hee I am measuing the length of each catilage gaft. This is based off of the length of the bidge of the patient that needs to be augmented. Hee I am timming the havested ea catilage fom the patient that I had havested ealie in the case. One of the benefits of using a patient's own catilage is that the gaft can be fully customized based off of the actual width that is needed and also the length. Thee is also the benefit, of couse, of using you own tissue. Thee is a much lowe isk of ejection o infection down the oad. Now that I have two layes of catilage based off of the septum and ea catilage I'll now laye o laminate them. They ae actually connected using sutues. In this case I'll be using a mixtue of both dissolvable, o esobable, sutues and also pemanent sutues. This is mainly in aeas that I want to ensue that catilage does not want to buckle, especially given the natual shape of the catilage in that aea. With time each laye of catilage will actually sca to each othe. This is typically done afte seveal months. The sutues ae mainly to ensue that thee aen't any changes duing the healing pocess. The actual thickness of the gaft is also dependent on the needs of the patient and thei desies. Some patients only need one laye of catilage if they only need minimal augmentation wheeas othe patients may need moe than one, up to two o thee. This patient will actually have a thee laye gaft ultimately using one laye of septal catilage and two layes using the ea catilage. You may have noticed that thee is actually additional septal catilage that we could have used. You may be wondeing why I'm not using it fo the bidge. The main eason is that I'm actually pefoming othe maneuves duing this hinoplasty on the tip of the nose in which I equie the stong, staight catilage of the septum. Ea catilage isn't typically as stong as septal catilage and has a natual cuvatue of the conchal bowl. This can be used on the dosum egadless. You can now see the gaft coming togethe, looking nice and staight. Once we'e done sutuing the layes togethe we'll then be able to bevel and tim the edges so that thee ae no shap edges. This is what we'e doing now with ou scalpel. As you can imagine the top of the bidge typically has a gentle cuve athe than a shap edge. We want to make sue this is continued with the gaft. Video tanscipt: "This is Seattle facial plastic sugeon, Docto Thomas Lampeti. Today I'm going to show you how we put on a hinoplasty dessing and cast. The fist step involves placing adhesive which allows the tape to stick bette to the skin. Next, I put on a Telfa dessing. This is a non-stick dessing that allows me to emove the dessing moe easily in a week. Next, we'e putting on stei-stip tape ove the Telfa. The goal of the tape is to keep down the skin swelling as much as possible. The tape also allows the skin to eseat against the undelying nasal stuctue as much as possible peventing any fluid o blood collection. Once the taping is complete we'll then cut to size the Aquaplast cast. Hee we ae with a sheet of the Aquaplast mateial which is basically a type of plastic. I cut it to the pope size fo the patient. And we'll then place in some hot wate which will tempoaily soften the cast. We'll then be able to mold the mateial to the patient's nose. Then within a few seconds the cast then hadens and potects the nose fom injuy and also allows the bones to heal in the pope position that we've set them in. And you can see hee that we have the Aquaplast in the hot wate. You can see it slowly becoming moe clea and that's showing us that it is becoming softe. When it is totally clea you can see that it is nice and moldable so we'll place it ove the tape, stetch it into position and then (this is all in eal time) you'll be able to see the plastic set and become white again. Then it will be quite had and povide nice potection fo the nose fo the next week until we take off the cast in the office. And you can see aleady that the cast in hadening. Facial fat gafting is a technique used in which a peson's own fat is havested and then injected into the face to combat the signs of aging elated to facial volume loss. Video Tanscipt: "This is Seattle facial plastic sugeon, D. Thomas Lampeti. Today I'm going to show you how we havest, centifuge, and pocess the fat fo facial fat tansfe. You can see how by hand I am liposuctioning this patient's lowe abdomen. The idea is to use a vey minimal amount of vacuum o suction in ode not to damage the fat cells. In this case we havested fou syinges woth of fat that we'e now putting in the centifuge. The idea is to sepaate the fat cells that we want fom the blood, lidocaine and fatty oils that would othewise hinde fat gaft take. I centifuge the fat fo 3 minutes at 3000 pm. While that is occuing we put stei-stips ove the havest site in the belly button. Once the fat has been pocessed you can see how it has been sepaated into the fat in the middle, the oils in the top and blood at the bottom. We pou out the oils which ae basically fom the uptued fat cells -- a minimal amount of oil is a good sign. Then the blood fom the bottom easily is dained. And then we tansfe all of the fat that has been isolated into one syinge. You can see how we have little cotton pledgets that help us wick any esidual oil. As it tuns out, oil is detimental to fat gaft -- they don't eally like that and it hindes fat gaft take. The last step befoe pefoming the fat gafting itself is to tansfe the fat into individual 1 cc syinges. These smalle syinges make it much easie to place the fat into the face in a vey guided and diect fashion. The vey small aliquots of fat that ae placed -- these vey small doplets -- ae moe easily done with a smalle syinges since less pessue is needed. The amount of fat that is needed fo any individual case is customized. So if only a small amount o one aea is going to be teated a smalle amount of fat would then be havested. Most people ae vey happy to donate exta fat just in case and so I do e on ove havest just to make sue that I have enough fat. This teminology comes fom the fact that we'e opening up the maxillay sinus and the antum is the window o opening of the sinus. Ou fist step is to pefom an uncinectomy. This is what we'e doing ight now as we incise the mucosa o skin lining ove the uncinate pocess. The uncinate pocess is the shelf of tissue that lays ight in font of the natual maxillay atium o opening. You can see just a pat of it thee as we make an incision in the uncinate. And as the name implies an uncinectomy involves emoving this tissue. So wee using a device to emove this potion of thin bone and skin lining. And now we'e using a shave o mico-debide to shave some of that excess tissue and fine tune the edges. When people conceive of sinus sugey they often think of a oto-oote pocedue and this device is pobably the most analogous to that. This view shows you the device and the shave blade. Thee is also iigation that goes though as it suctions also. And this allows us to emove the tissue that we need to emove, including polyps in cetain cases, while spaing any mucosa o skin lining that we don't want to injue. This device is called a back-bite. You can see why we call it that. This is allowing me to emove some additional tissue and open up the cheek sinus futhe. And now we'e placing some dissolvable packing. In most cases in my pactice this is the extent of the packing that I'll place duing sinus sugey -- and it is dissolvable. As you can see thee is still quite a bit of oom still in the nose and you'e still able to beathe though you nose if you like. And this packing will dissolve ove time and you do salt wate inses and flushes. This packing has two oles. This individual had diffuse sinusitis despite previous surgery. In this video demonstration I show left sphenoid sinus balloon sinuplasty performed in the office in a patient with chronic left sphenoid sinusitis. The sphenoid sinus is one of four sinuses or air spaces within the facial bones and is located all the way towards the back of the nose. The sinus opening was completely occluded with edema and the initial probing identified the opening under endoscopy. Once the opening is identified , the balloon probe is advanced into the opening and the balloon then inflated and deflated a number of times to enlarge the opening to about 6 mm size. Secretions or pus can then be suctioned out and even polyps or fungus removed, all under local anesthesia.

This is a demonstration of surgery results immediately after performing total sinus surgery with balloon sinuplasty in the office under local anesthesia and some oral sedation. The ethmoid sinuses between the eyes have been widely opened with a shaver. This new retractor enables the application of antimetabolite during trabeculectomy surgery. Whilst the antimetabolite soaked pledgets are inserted, the retractor atraumatically holds the conjunctival edge away and supports itself on the sclera to relieve surgeon fatigue. It is recommended to avoid pooling of antimetabolite on the top of the retractor by keeping the corrugated surface dry. After the pledgets are removed at the end of the specified time, the antimetabolite is irrigated out with BSS and the subconjunctival space examined to avoid any retained pledget material. It comes in right (01 09528) and left-handed versions (01 09529) from John Weiss. The retractor is usually held by your non-dominant hand.

Performed by Kami Parsa M.D. Patient is a 55 year old with a history of previous upper eyelid blepharoplasty with excessive skin removed from both upper eyelids which resulted in bilateral lagophthalmos. Patient could not close her eyes and had problems with severe dry eyes.
